Insurance & Billing
As stated in the Financial Policy, the providers at Elevated Natural Health are not preferred medical providers at this location for any insurance provider. Elevated Natural Health will provide you with the required forms to submit to your insurance at the time of checkout. We recommend you contact your insurance provider prior to your first appointment to review your plan coverage. To determine if you will get reimbursed for your visit, you need to ask if you have “out of network coverage for naturopathic physicians.” You may also want to ask at this time how to submit your forms for reimbursement after your visit. Every insurance company will be slightly different, but many patients get a large portion of their office visit reimbursed by their insurance.
*Please note that regenerative injection treatments (Prolotherapy, Prolozone, PIT, Neural Therapy, PRP, microneedling, etc.) are NOT covered by insurance. You should plan to pay for these treatments entirely out of pocket.
Rates
Please contact us for an itemized and updated menu of services and prices. Prices listed here are an estimate and subject to change at any time. Each type of visit has a price range since the final price is dependent on time, complexity of the case, and a few other factors.
Initial office visit: $280 for basic and brief intakes; up to $375 for thorough, comprehensive intakes
Return office visit: $198 for ~30 min; up to $275 for ~60 min
Phone consults: $121 for a brief ~20 min consult; up to $275 for ~60 min consult
